This study supports cautious off-label use of cefiderocol as salvage or targeted therapy for severe MDR/XDR Gram-negative infections in children and neonates, particularly when alternatives are lacking. However, the evidence remains low quality (case series/case reports), and prospective pediatric PK and efficacy studies are still needed before routine use can be broadly recommended.
